Temporary health insurance covers a wide variety of needs, from 30 to 365 days (depending on state of residency), and it allows you to use your own doctors and hospitals.

When you need affordable health insurance and are between permanent employer-sponsored plans, temporary health insurance offers gap coverage that can give you and your family peace of mind. Temporary health insurance policies cover physician services, prescriptions, X-ray, laboratory services, inpatient hospital stays, inpatient and outpatient surgeries, skilled nursing facility care and rehabilitation up to a lifetime maximum of $2 million.

    General Information

    Q.When does my coverage begin?

    A.If you are submitting your application by:

    Internet using a credit card - The earliest your coverage can begin is the day following transmission, if all other eligibility criteria have been met. For example, if you submit your application online on March 16th, your coverage begins at 12:01 A.M. on March 17th. Dates of the 29th, 30th and 31st are not available.

    All transmissions take place and are recorded based on the time and date in the Central Time Zone. For example, if you submit your application on-line at or after 10:00 P.M. on March 15th from a location in the Pacific Time Zone, the time of the transmission will be at or after 12:00 A.M. Central Time. The transmission date of your application will be March 16th.

    Mail and writing a check - The earliest that your coverage can begin is the day following the U.S. Postal Service postmark, if all other eligibility criteria have been met. (If the envelope containing your application is not postmarked by the U.S. Post Office or if the postmark is not legible, coverage will begin the later of a) your requested date or b) two days prior to the date the application was received by Assurant Health.) Dates of the 29th, 30th and 31st are not available.

    Coverage will take effect provided the following conditions are met: Your completed application and full premium payment are received by Assurant Health and you meet the requirement for acceptance. 

     

    Q. What is the definition of a pre-existing condition?

    A. A pre-existing condition is an illness or injury for which the covered person received medical advice, diagnosis, treatment or services from a physician within the 12 month period prior to the covered person's effective date; or that produced signs or symptoms within the 12 month period prior to the covered person's effective date.
     

    Q. How many times can the plan be renewed?

    A. Student Select is renewable as long as you need it...and, of course, as long as the premiums are paid. This is important after graduation while looking for a job.

    Q. When will I receive confirmation or acceptance of the application?

    We will review the application for eligibility and check the submitted premium.

    If you apply via the Internet, you will receive an email message with confirmation or acceptance of the application shortly after it is submitted. Your identification card and contract will be sent to the "correspondence address" indicated within approximately 72 hours of receipt.

    If you print out the application and mail it to us, most eligibility determinations are made within 48 hours of receipt. If the application is complete and the full premium is paid, the identification card and contract will be sent to the "correspondence address" indicated within approximately 72 hours of receipt.

    Please Note: If you are required to provide proof of insurance to your school, please submit the application at least two weeks before proof of coverage is needed.

    Q. If I graduate or drop out of school, will I lose coverage?

    A. No, your Student Select policy stays with you for as long as you need it and, of course, as long as premiums are paid.

    Q. Will coverage be in jeopardy if I drop a class?

    A. No, not as long as:

    1) You met the definition of an eligible student on the date the application was signed.

    2) You attended school full-time for 31 days after the policy effective date.

    3) The premium is paid in full.





    Eligibility

    Q. Who is considered an eligible student?

    A. An eligible undergraduate student is defined as a person carrying at least nine credit hours. An eligible graduate student must meet the graduate student guidelines of the college or university for full-time student status*. Students must attend a state-accredited college or university. Please check with your college or university to see if the school is listed in the 2001 Higher Education Directory. The student must be under the age of 30 and be able to answer "NO" to the medical questions on the application.

    You must be a full-time student for a minimum of 31 days following the effective date of your policy.

    * Assurant Health will accept the school's definition of full-time.


     

    Q. Some schools operate on a quarterly schedule. Full-time undergraduate status is six to eight credits per term. Do I qualify for Student Select?

    A. Yes. You should tell us in writing (attached to the application) that you are attending a school with a quarterly class schedule and are meeting the school's definition of full-time status.

    Q. I am a foreign exchange student. Am I eligible for Student Select coverage?

    A. Yes, as long as you meet the eligible student status requirement defined above.

    Q. Can I add my family members (spouse and dependents) to my policy?

    A. The Student Select plan is designed for individual college students only and does not cover spouses and/or dependents. Student Select does, however, comply with any state requirements for newborn coverage.

    Q. When can I apply for a policy? I would like to apply now but won't be a student for another month?

    A. You can apply as soon as you are enrolled to be an eligible student.

    Q. How far in advance can I apply if requesting a later effective date? In other words, can I apply in July but request an effective date for September 1st when school begins?

    A. Yes, but keep in mind the full premium must be paid with the application. The policy will be sent out as soon as it is issued. We would prefer that the application not be completed more than 60 days prior to the requested effective date.





    Benefits

    Q. Are there additional benefits available?

    A. Maternity benefits are available in most states for an additional charge. Additional benefits such as psychiatric, substance abuse/mental illness, alcohol, and rehabilitation vary by state.

    Q. Does the Student Select plan cover me when I go out of state?

    A. The Student Select plan travels with you anywhere in the United States, its possessions and Canada. And, although it does not cover you in a foreign country, Student Select does cover you for an emergency medical evacuation to the home country or a facility operating within its laws.

    Q. Are prescription drugs covered?

    A. Prescribed drugs are covered if you are hospitalized as an inpatient. Since outpatient drugs are provided through most university health centers, they are not a covered benefit with Student Select. This helps keep the plan affordable.

    Q. How often does the deductible start over?

    A. With the Student Select plan, the deductible is a "calendar-year" deductible. This means it starts over again on the first day of each new calendar year. The Student Select plan includes a carryover deductible feature that looks at any covered charges you had in October, November or December of the previous year that goes toward meeting the deductible. We will carry over the full amount of these charges to the next year to help meet the new calendar-year deductible.





    Preauthorization

    Q. Is preauthorization required with a Student Select policy?

    A. Yes. An authorization service is used which ensures you receive the most appropriate and cost effective care available. Trained staff work with you and your physicians to review the course of treatment and advise you of your eligibility for benefits. The identification card you receive with your policy provides a toll-free number for easy access to this service. The authorization process must be followed in its entirety to receive maximum benefits. The policy explains the authorization process in detail.
